Welcome to Medivive. This Telehealth Consent (“Consent”) outlines the terms and conditions under which Medivive, through its healthcare providers, offers telehealth services. By using our telehealth services, you agree to the terms of this Consent and acknowledge the potential benefits and limitations of receiving healthcare remotely.

1. Understanding Telehealth Services

Telehealth involves the use of electronic communications to enable healthcare providers to deliver healthcare services to patients who are not physically present with the provider. These services may include medical history review, consultation, diagnosis, treatment, follow-up, and patient education. 

2. Benefits of Telehealth

  •  Access to care from the comfort of your home or another convenient location.
  •  Reduced need for travel, which can save time and reduce the risk of exposure to contagious diseases.
  • Ability to receive timely care, particularly in situations where in-person visits may be difficult.

3. Potential Risks of Telehealth

  • In some cases, telehealth may not be as effective as in-person visits, particularly if a physical
  • Technical issues such as poor internet connection or equipment failure may disrupt the consultation or delay medical advice.
  • There may be limitations in the ability of the provider to fully diagnose or treat certain conditions remotely, and an in-person visit may be recommended or required.

4. Your Responsibilities

  • You agree to provide accurate and complete information about your health to the best of your ability.
  • You will ensure that you are in a private, secure environment during your telehealth consultation to maintain confidentiality.
  • You agree to follow the provider’s advice, including seeking follow-up care or in-person consultation when recommended.

5. Privacy and Security

Medivive is committed to protecting the privacy and security of your health information. All telehealth consultations will be conducted using secure, encrypted platforms to protect your personal health information. However, there is a small inherent risk that unauthorized third parties may access the information.

6. Consent to Telehealth Services

By agreeing to this Consent, you acknowledge and agree to the following:

  • You understand the nature of telehealth services, including the benefits and potential risks.
  • You consent to receive healthcare services via telehealth, including consultation, diagnosis, and treatment.
  • You understand that telehealth services are not a substitute for emergency care, and in the event of a medical emergency, you will call 911 or go to the nearest emergency room.
  • You understand that you may withdraw your consent to receive telehealth services at any time by notifying Medivive in writing or verbally during a consultation.

7. Limitations of Telehealth Services

While telehealth can be an effective tool for many types of healthcare, it may not be suitable for every medical condition. Medivive providers may determine that your condition requires an in person visit, and if so, they will advise you accordingly.

8. Financial Responsibility

You are responsible for any fees associated with telehealth services as outlined in Medivive’s pricing policy. Payment is required at the time of booking, and you agree to adhere to the payment and refund policies in place.
